Notice to Prospective Mass Attendees

It is a requirement of the Australian Government that everyone who attends Mass must complete all the required details on the Contact Tracing Register sheets on arrival.

Failure to do so could result in a fine being issued to the Parish in the event that a person or persons later contracts COVID19. We must keep this Register in a secure place for 56 days.

By signing this form, you acknowledge that:

*You do not have any symptoms associated with COVID19 – cough, sore throat, shortness of breath, sneezing/runny nose or loss of sense of smell.

*You do not have COVID19 nor are you awaiting the results of tests for COVID19.

*You have not been in contact with any known or suspected cases of COVID19 in the past 14 days.

*You have not returned or been in contact with anyone else who has returned from overseas in the past 14 days.

If you answer ‘yes’ to any of the above or you are feeling unwell, please do not stay. You are most welcome to return when your health has improved.

Please use the hand sanitisers available and maintain the required 1.5 metre distance during Mass.
